Coordinates continuity of care from acute hospitalization to a lower level of care based upon the patient s needs to insure best possible outcomes and appropriate utilization of community resources. JOB SPECIFICATIONS Education and/or experience: Bachelors Degree in social work or related field required; two to four (2-4 years) experience in an acute care setting preferred. Knowledge: Discharge planning process including community resources, insurance regulations (Medicare and/or Medicaid) as applicable to a sub-acute level of care. Skills: Intermediate computer skills. Good oral and written communication skills; organizational skills required.
