Community Health Partner (CHP) Manager

Location: Remote, field-based (must reside within a designated market)

Department: Clinical Operations

Employment Type: Full-Time


Join Matter Health as a CHP Manager, where your leadership will directly impact underserved populations by supporting Community Health Partners in driving access to high-quality, preventive care. This is a field-based leadership role with regular travel across assigned residential and partner sites.


What You'll Do

  • Lead, coach, and supervise a team of Community Health Partners (CHPs) across your assigned market
  • Drive team performance through 1:1s, field shadowing, ride-alongs, and data reviews
  • Monitor patient panel outreach, preventive screenings, and post-discharge follow-up workflows
  • Analyze performance data to identify trends and coaching opportunities
  • Support hiring, onboarding, and training of new CHPs, including coordination of medical assistant skills training as needed
  • Collaborate cross-functionally to refine workflows and streamline care coordination
  • Serve as subject matter expert for the CHP role and provide day-to-day support
  • Navigate and manage escalations related to properties, community stakeholders, or patient concerns
  • Champion compliance, quality, and continuous improvement initiatives
  • Communicate key updates and organizational priorities to CHPs and leadership

What You Bring

  • Bachelor's degree in Public Health, Health Administration, Social Work, or related field (or equivalent experience)
  • 2–3+ years of leadership or supervisory experience, ideally in primary care, community health, senior care, or hospital settings
  • Experience working with medically complex, underserved, or senior populations preferred
  • Proven ability to coach, develop, and manage high-performing teams
  • Comfortable navigating EHR, CRM, and care coordination tools
  • Strong data analysis, problem-solving, and performance tracking skills
  • Excellent communication, interpersonal, and conflict resolution abilities
  • Ability to travel frequently within your designated market and work in diverse physical settings (indoors and outdoors)


Physical & Travel Requirements

  • Field-based role with frequent travel to residential properties and community locations
  • Must have a valid driver's license and be able to lift up to 50 lbs
  • Comfortable climbing stairs, navigating outdoor settings, and engaging with patients in varied environments and weather conditions
