Responsibilities:

The Integrated Care Counselor plays a crucial role in delivering therapeutic services and care coordination aimed at enhancing clients' mental and physical health. This position involves:

  • Conducting thorough biopsychosocial assessments to identify individuals' needs.
  • Creating and implementing personalized treatment and care plans in collaboration with clients and their families.
  • Providing individual therapy sessions and therapeutic interventions for various mental health challenges, including anxiety, depression, trauma, and behavioral issues.
  • Maintaining accurate documentation of client interactions and progress in accordance with company policy.
  • Collaborating with multidisciplinary teams to ensure holistic care and participate in treatment team meetings.
  • Engaging in community outreach to promote our services and attract new clients.
  • Coordinating care during transitions and connecting clients with essential resources and support services.

Qualifications:

Candidates must meet specific requirements based on their state of practice. For North Carolina, a Master's Degree in Social Work or Counseling is required, along with an associate license as a LCSWA or LCMHCA. For Tennessee, candidates should have a Master's Degree in a related field and must be eligible for licensure as an LPC, LMFT, or LCSW, with a minimum of two years of experience working with children and families.
