Patient Flow Coordinator

Description:

Position Summary:
Provides mobile lung screening services for patients and providers throughout Knox County and surrounding counties.
Performs patient intake and registration for each clinical site.
Builds strong relationships within the community.
Supports Community Health Improvement as well as other Population Health strategic initiatives.
Service
Performs various duties
a. Demonstrates flexibility in work schedule.
b. Demonstrates competency in all computer software used for daily operations: Cerner registration, scheduling, Power Chart, Tracker, PACS, and document scan
c. Arrives to all sites one hour prior to first patient start time.
d. Maintain all required tracking logs daily.
Provides friendly customer service
a. Receives zero justified customer complaints.
b. Greets each patient with a focus on providing compassionate, professional care.
c. Leads by example and demonstrates effective teamwork.
d. Assists patients with any physical need.
Registration and management of records
a. Demonstrates competency and accuracy with patient registration and insurance verification processes.
b. Demonstrates knowledge of insurance providers, financial guidelines and available financial resources.
c. Ensures that patient chart is prepared for date of service.
d. Verifies all patient information to include updated phone numbers, current insurance and orders if indicated.
Efficiency and Effectiveness
Accurate and fast interaction via computer.
a. Types 50 wpm within probation period.
b. Typing is accurate and with minimal errors.
Communication via telephone and in person
a. Communication via telephone and in person.
b. Documents all required information in Cerner Scheduler.
c. Completes Progress Notes for documentation of clinical information and follow-up information.
Coordinates and prioritizes daily activities and completes all essential job tasks
* Zero uncompleted essential job tasks.
* Collaborates with manager and team for ongoing process improvement to support patient and provider relationships
Quality and Safety
Receives complex information. Processes, synthesizes, and formulates correct action
a. 90% accuracy within probationary period.
b. Navigates departmental communication tool (sharepoint) regularly to maintain current knowledge of protocols, changes, events and schedules
Position Qualification:
EDUCATION, EXPERIENCE
* High School Diploma or GED Required
* 1 to 2 years of clinical or non-clinical healthcare related experience
