Description:

The Home Care Support Specialist is a dedicated professional providing essential personal care and related services directly in clients' homes. Working under the guidance and supervision of the Director of Nursing or RN Team Leader, you'll play a vital role in enhancing the well-being of those you serve.

QUALIFICATIONS:

  • Knowledge of home health care programs and prior experience with home health visits is advantageous.
  • Ability to read and write at a level consistent with the job requirements.
  • Completed PCA training covering:
    • Methods for aiding clients in achieving greater self-reliance.
    • Nutrition principles and meal preparation.
    • Understanding the emotional challenges related to illness and long-term home care.
    • Maintaining a clean, safe, and welcoming living environment.
    • Reporting any changes in client condition to nursing staff.
    • Upholding ethics and confidentiality in client care.
    • Collaboration with the healthcare team.
    • Record-keeping duties as required.
  • Basic skills in personal care, housekeeping, and meal prep.
  • A genuine interest in and concern for others.
  • Good hygiene practices and a professional appearance.
  • Health-free from issues that may pose risks to clients, yourself, or co-workers.
  • Some relevant work experience.
  • Emotional maturity necessary for fostering positive relationships with clients and their families.
  • Reliable transportation and a valid TN Driver's License.
  • Auto insurance as mandated by the agency.
  • A clean driving record.

KEY RESPONSIBILITIES:

  1. Assist clients in maintaining personal hygiene.
  2. Help maintain a safe and healthy environment.
  3. Plan and prepare nutritious meals, including shopping and errands as directed by the supervisor.
  4. Support client ambulation and mobility.
  5. Encourage client independence according to their nursing care plan.
  6. Promote mental engagement through activity involvement.
  7. Provide emotional and psychological support to clients and family members.
  8. Build trusting relationships based on confidentiality.
  9. Document visit reports daily and incorporate them into clinical records.
  10. Notify supervisors of any changes in client conditions or home environment.
  11. Follow assigned instructions and communicate when unable to complete tasks.
  12. Collaborate with other agencies involved in client care as needed.
  13. Perform housekeeping tasks related to creating a safe and comfortable space for clients.
  14. Adhere to universal precautions during care procedures.
  15. Safeguard confidentiality regarding all client and organizational information.
  16. Provide only specified functions for each individual client.
  17. Respond to the physical, emotional, and developmental needs of clients.
  18. Follow emergency protocols as needed.
  19. Understand and comply with all organizational policies and procedures; seek clarification when necessary.
  20. Provide feedback on team members' performance when requested.
  21. Attend in-services and meetings as assigned by supervisors.
  22. Maintain a positive, professional attitude while working with staff to foster teamwork.
  23. Notify supervisors in advance of any absence or tardiness, without abuse.
  24. Keep personal matters from interfering with scheduled work hours.
  25. Present a professional appearance at all times.
