Pediatric Community Health Worker

Monday to Friday 8:00am - 5:00pm (no weekends, nights, or major holidays)

Full-time (40/hour per week)

Locations available: Knoxville, TN 37919, In-Person

Overview
The Community Health Worker (CHW) supports pediatric patients and caregivers by providing outreach, education, care coordination, and referrals. This role works closely with care teams and community partners to reduce barriers to care, promote preventive services, and improve access for under-resourced families.

Key Responsibilities

  • Conduct outreach, education, follow-up, and care coordination
  • Connect families to RVH and community resources
  • Support engagement in preventive and recommended pediatric care
  • Collaborate with care teams and community partners
  • Assist with documentation, reporting, research, and quality improvement activities
Qualifications:
  • High school diploma required; Bachelor's degree preferred
  • 1-2 years of experience connecting patients to community resources preferred
  • Ability to speak to individuals on 1:1 basis, in family groups, and in treatment groups, to engage potential community partners, to disseminate information about activities and outcomes
  • Experience with children ages 0-5 and caregivers
  • Knowledge and training of early childhood development, including autism spectrum disorders and adverse childhood preferred
  • Strong communication and digital skills
  • Knowledge of Word Processing software, strong digital literacy and health literacy
  • Spanish fluency preferred
