Covenant Health - - Responsibilities: Meet with the entire team weekly to review treatment needs of consumers; Contribute to development of comprehensive care plan for designated service participants; Work with service participants and their families to understand and obtain health care services and develop a healthy lifestyle; Assess life domain needs of consumers across various areas (living arrangements, finances, health, etc.); Provide ongoing linkage, monitoring, planning and evaluation, and advocacy for team consumers
