Prisma Health - - Responsibilities: Provide constant 1:1 observation of assigned patient; Document and report risks or changes to prevent self-harm and harm to others; Perform safety sweeps when observing patients at risk for self-harm; Assist with turning/positioning, diversional activities, feeding, ambulation, transfers and transports; Attend to patient needs during bathing, toileting, and sleeping while under observation
