Prisma Health - - Responsibilities: Provide constant 1:1 observation of assigned patient; Document and report risks, changes or problems observed to prevent self-harm and harm to others; Complete safety sweeps when observing patients at risk for self-harm; Ensure patient is not unattended and engage unit staff or Security if patient tries to leave the floor; Give and receive hand-off communication regarding patient assigned to observe; Assist with meeting patient's needs including turning/positioning, diversional activities, feeding, ambulation, transfers and transports; Be in attendance with the patient at all times including bathing, toileting, and sleeping
