This unique role blends nursing knowledge with scheduling and care coordination, helping our teams deliver exceptional home health experiences every day.Position Highlights Manage and coordinate patient referrals, visit schedules, and start-of-care assignments using an automated scheduling system.Use your clinical background to support appropriate scheduling decisions, ensuring care plans align with patient needs and regulatory standards.Partner daily with field clinicians, case managers, and office teams to resolve any pending or incomplete visits and maintain continuity of care.Monitor and update scheduling workflows for new, missed, rescheduled, and reassigned visits, promoting accuracy and patient satisfaction.Must Haves At least one (1) year of experience in a healthcare office setting, such as admissions, patient access, or intake, with experience in home health or hospice preferred.Demonstrated experience or ability to quickly learn and utilize an Electronic Medical Record (EMR) system; familiarity with Homecare Homebase (HCHB) is a major plus.Basic knowledge of Medicare, Medicaid and commercial insurance plans and terminology related to eligibility and authorization.Strong understanding of medical terminology and documentation requirements.Knowledge of computer and electronic HRIS and EMR systems a plus.Detail focused and ability to set appropriate priorities amongst multiple demands.Knowledge of state and federal employment compliance regulations preferred.A positive, flexible, and caring personality who displays good judgment, inspires confidence, and handles confidential matters with discretion.Excellent listening skills and demonstrates professional empathy.Must consistently demonstrate autonomy, organization, efficient time management, confidence, flexibility and cooperation in performing job responsibilities.Full-Time Benefits Medical, Dental, Vision, Life Insurance, Disability, Pre-Tax Savings Accounts, and ancillary benefits 100% Employer Paid Basic Life Insurance & Employee Assistance Program Generous Paid Time Off plan and 6 paid holidays annually Employee Referral Bonuses 401K Retirement Plan & Employer MatchEmployer Information UT Home Health, an affiliate of The Pennant Group (NASDAQ: PNTG), is the employer for this position as stated in the job posting.
